Parent Consent Agreement - Must Read

Informed consent and Acknowledgment- I hereby give my approval for my child's participation in any and all activities prepared by Kidspace Learning during the selected programme. I assume all risk and hazards incidental to the conduct of activities, and release, absolve and hold harmless Kidspace Learning representatives from any and all liability for injuries to said child arising out of travelling to, participating in, or returning from programme. In case of injury to my child, I hereby waive all claims against Kidspace Learning. 

Medical Release and Authorisation: 

As Parent and/or guardian of the child I am booking in, I hearby authorise the diagnosis and treatment by a qualified and licensed medical professional , of the minor child, in the event of a medical emergency whereby I cannot be contacted.  In the event of emergency and in my absence I agree the Kidspace Learning representatives to contacting Emergency Services.

Refund Policy - I understand and agree that there is no refund policy in respect of this programme unless the event is called by Kidspace Learning.

Information provided on this form will be used for development of programmes and to update you as relevant, we will not disclose personal information to third parties unless we believe, in good faith, that we are required to disclose to comply with any applicable or statutory requirement.
